ACCREDITATION/ENTRY PROCESS

1.         Accreditation of Entries by Name of participating athletes shall be processed at the closest AMA branch.

2.         Athletes must be accompanied to AMA by their respective NSA provincial coordinator or the LGU sports coordinator.  Underage athletes must also be accompanied by at least one parent or legal guardian.

3.         Athletes must bring with them a certified true copy of their birth certificate or any valid ID that will prove their age and identity.

4.         Should the NSA not have a coordinator in a particular area, the NSA must provide the POF Organizing Committee with a list of athletes from that area who will compete in the Games.  AMA will only accept registrants for accreditation in the NSAs official list.

5.         The athlete fills out and submits two (2) signed copies of the Entry Form by Name and has his/her photo taken by AMA, if he/she does not have one.  The information will be uploaded into the POF website by AMA.

6.         AMA will return one hard copy of the Entry Form by Name to the athlete.  The second hard copy will be kept for POF files.

7.         The NSA will verify and screen all entries in the POF website.  If more than the maximum allowable number of athletes and officials has registered for the competition, it is the NSAs responsibility to decide which of the entries to accept.  The corresponding box beside the athlete’s name should be marked. It is the NSAs responsibility to inform an athlete or official that they have been rejected.  The NSA will electronically forward the confirmed list of entries to the POF Organizing Committee.

8.         The POF Organizing Committee is responsible for informing AMA of the confirmed list of entries for each sport.

9.         AMA will print AD/ID cards based on the confirmed list forwarded by the POF Organizing Committee.

10.        Completed AD/ID cards will be released to the NSAs for distribution before the Games.
